I've seen this several times too. I think this is what happens: the monsters will decide, for some reason, that they want to be in a square past the party. So they'll decide to "go around" your obstructing group somehow and move a step away from you. Then, when attacks are resolved, they see a target (i.e. your group) 2 steps away so the choose "ungroup" toward the party. Thus, ending up right next to you where they started. Of course, they used their action to "ungroup" and, of course, they were not adjacent when the combat turn began so your party could not choose to attack as an action. Thus at the end of the turn it looks like nothing has happened since you don't see the "in between" step of them moving away from you and neither side has any opportunity for an attack.

There are several really dumb things monsters are doing when it comes to deciding how to engage the PCs. This is surely one of them. I've "solved" it two ways. The easiest, as you mention, is to just ungroup into their square too. Then you all end up in the same place and can fight next turn. Alternately, if you back off a few squares they'll usually be able to "get where they're trying to go" by following you back and then things return to normal.

Please add more dumb monster things. We do enough dumb player things over clicking, forgetting to switch characters to attack after equipping, leaving caster on same casting action long past point needed to be done.. So give monsters more dumb stuff.. think I asked for this years ago but have racial enemies/monsters fight each other.. ie. orcs v elves, demons v devils etc
